…give my permission for my child/children to be filmed/photographed/interviewed by the media during church events and for church or Presbytery to use my child’s photograph/work/ voice for promotional and education purposes, including church and Presbytery web sites.

Cost: $15 per child with a $40 max per family (scholarships are available).  Please mail or bring your check or cash into First Presbyterian Church (4821 Bloom Ave. White Bear Lake, MN 55110).  Checks can be made out to First Presbyterian Church.

Registration deadline is Monday, July 15th.
